aglaseroffChronic disease is reponsible for more than 75 percent of the nation's health-care costs. In an effort to improve patient care and reduce these costs, a new Stanford clinic will help patients with serious, chronic problems avoid health crises. The clinic, which is scheduled to open in April 2012, will provide focused attention from a primary care team for patients with severe chronic disease, such as uncontrolled asthma, diabetes or heart disease. In this podcast, Alan Glaseroff, MD, director of the clinic, talks about how he hopes it will serve as a new model for patient-centric, value-based medical care. Length: 28 min.
